The Liver-Scarring Score Hidden in Routine Bloodwork for Men Over 40

Normal AST and ALT results can still miss liver scarring. Learn what the FIB-4 score estimates from four numbers you may already have, and when to ask about it.

Imagine opening the results from your annual physical. Your AST is not flashing red. Your ALT is not flashing red. Nothing on the liver panel looks dramatic, so you close the page believing your liver has been cleared. But those two enzyme results may not answer the question you think they answer. AST and ALT can signal liver-cell injury. They do not directly measure the amount of scar tissue in the liver, and in some people with clinically significant liver disease, these enzyme levels can remain within a laboratory's reference range. Here is the surprising part: the same routine results may already contain four clues that can help estimate the risk of advanced liver scarring, your age, AST, ALT, and platelet count. Put those four values into a calculation called FIB-4, the Fibrosis-4 index, and a routine blood draw becomes a first-pass liver-fibrosis risk assessment. Not a diagnosis. Not a biopsy. A sorting tool.

Fat is common. Scarring changes the stakes.

The modern liver story often begins with metabolic health. The condition once called nonalcoholic fatty liver disease is now called metabolic dysfunction-associated steatotic liver disease, or MASLD. The new name connects fat in the liver with cardiometabolic risk factors such as excess weight, type 2 diabetes, prediabetes, abnormal cholesterol or triglycerides, and high blood pressure.

Fat in the liver is common, especially among people with type 2 diabetes. But fat is not the only question that predicts what happens next. The more consequential question is whether ongoing injury has triggered fibrosis, the accumulation of scar tissue.

The liver can repair damage, but repeated injury can turn repair into remodeling. Imagine a flexible sponge gradually crossed by stiff bands. Early fibrosis may cause no obvious symptoms. As scarring advances, the liver's architecture and blood flow can change. At the far end is cirrhosis, where severe scarring raises the risk of liver failure, liver cancer, and other complications.

That progression is not inevitable. Many people with liver fat will never reach advanced fibrosis, and nobody can look at body size, symptoms, or one enzyme result and know where they stand. This is why clinical pathways focus on risk stratification: use an inexpensive first step to identify the large group unlikely to have advanced fibrosis and the smaller group that deserves a closer look.

For adults with type 2 diabetes or prediabetes, especially when obesity or other cardiometabolic risks are present, the American Diabetes Association's 2026 Standards of Care recommend FIB-4 screening even if liver enzymes are normal. That does not mean every man who turns 40 needs a FIB-4 score. It means age often arrives alongside the risks that make the score useful.

Why normal liver enzymes can be false reassurance

AST and ALT are often described as liver tests. That shorthand is convenient but incomplete. ALT is concentrated in liver cells, while AST is also found in muscle and other tissues. When cells are injured, these enzymes can leak into the bloodstream. An elevated result can be a clue that something is happening.

But a liver enzyme is not a scar meter. The amount of enzyme in the blood does not map neatly onto the amount of fibrosis. Results can fluctuate. Reference ranges vary between laboratories. Exercise, alcohol, medications, infections, muscle injury, and other conditions can influence the numbers. Advanced metabolic liver disease can sometimes exist with aminotransferases that are not elevated.

That creates a common reasoning error: 'My AST and ALT are in range, therefore my liver has no significant disease.' The safer statement is: 'These enzymes are not showing a clear warning today, but their meaning depends on my risks and the rest of the evaluation.'

Routine ultrasound has a similar limit. It can show fat, especially when enough has accumulated, but an ordinary ultrasound cannot reliably tell you how much inflammation or fibrosis is present. Clinicians needed a practical bridge between a basic laboratory panel and more specialized testing. That bridge is FIB-4.

The four numbers and the formula

FIB-4 was originally developed in people with HIV and hepatitis C, then studied and validated across other chronic liver-disease populations, including metabolic liver disease.

The formula combines your age at the time of the test, your AST, your platelet count, and your ALT. Age enters the calculation directly, which is one reason the same enzyme numbers carry different weight at 45 than at 70.

  • Age at the time of the blood draw.
  • AST (aspartate aminotransferase).
  • ALT (alanine aminotransferase).
  • Platelet count from the same routine panel.

What the three risk zones actually mean

For many adults younger than 65 being assessed for metabolic liver disease, the common pathway uses three zones. Below 1.3 is generally a lower-risk category, where periodic reassessment is often appropriate. From 1.3 to 2.67 is an intermediate zone that usually leads to a second, more specific test. Above 2.67 is a higher-risk zone that warrants specialist evaluation.

Age changes the cutoffs. For adults 65 and older, a higher lower-zone threshold is commonly used, because age is part of the formula and a fixed threshold would overflag older adults. If you are over 65, acutely ill, or have another reason the inputs may be unreliable, ask your clinician which threshold and timing apply to you.

The important concept is sequence. FIB-4 is a broad, inexpensive first step. When it is not low, the follow-up is usually a liver-stiffness measurement called elastography, often completed in minutes, or a blood panel such as the ELF test that measures proteins related to fibrosis. In more complex or uncertain cases, specialists may use magnetic resonance elastography or occasionally a liver biopsy.

What to ask at your next appointment

A focused appointment question could sound like this: 'I have type 2 diabetes and my AST, ALT, and platelet count were checked. Has my FIB-4 been calculated, and would the result change whether I need a liver-stiffness test?'

Or: 'My ultrasound mentioned fatty liver. Have we assessed my risk of fibrosis, rather than only the amount of fat?'

Those questions are better than demanding a scan. They connect the test to a reason and leave room for the clinician to consider age, acute illness, other causes of abnormal results, and local guidelines.

What FIB-4 cannot tell you

Before using any risk score, ask what it cannot do. FIB-4 does not diagnose fibrosis, MASH, or cirrhosis. It estimates the probability of advanced fibrosis. An intermediate or high score is a reason for confirmation, not a verdict.

The score can also be distorted. Acute illness, heavy recent exercise, certain medications, and conditions that change platelet counts can all push the number in either direction. Units must be correct, the values should generally come from the same blood draw, and the result must be interpreted in the right clinical setting.

The practical path is simple. First, understand your risks: type 2 diabetes, prediabetes, obesity, high blood pressure, abnormal lipids, cardiovascular disease, alcohol exposure, or fatty liver on imaging. Second, get the actual values from the same blood draw rather than relying on 'your labs look okay.' Third, interpret the result as a pathway. Fourth, confirm rather than catastrophize. That question does not diagnose you. It opens the right door.
